Incident Description:
I plugged 2 tools into a Husky (by home depot) extension cord. upon plugging in the 2nd tool, the cord started arcing and caught fire and neither tool was on. Both tool cords had the prongs melt and I received 2nd degree burns to my hand. It seems the cord end was not constructed correctly for the 2 cords to have some how made contact for arcing.

Incident Description:
While I (a 64 year-old adult with an engineering degree) was using the [REDACTED] to cut replacement pet window screening, the cutting blade (Stanley 11-983) pivoted within the tool handle and pried open the tool handle exposing the users hand to the opposite end of the cutting blade. Eventually the blade completely dislodges from the tool. No injury occurred as it was obvious the blade was not cutting well due to the changing angle of the blade.

Incident Description:
Purchased Cuisinart brand grilling tools from TJMaxx, Rossford OH. While using the turner/spatula to press down on hamburgers, tool broke where the body of the tool meets the handle. Tool dropped onto hot grill, and hand came into contact with surface of grill (I survived!). Examining remains of tool, it appeared to be snapped together, not very sturdy, in my opinion.

Incident Description:
husky tool chest combination model #H52CH6TR9HD Top widest drawer of lower cabinet is attached with flimsy slides that will not support weight when loaded with tools.
After loading the drawer fell off when opened and the tools fell out. I was not injured, but I could see where this could be very hazardous depending on the situation. I believe the drawer slides and the design are flawed.

Incident Description:
I purchased this "Dremel 7700-1/15 MultiPro 7.2-Volt Cordless Rotary Tool Kit" from Amazon in November, 2013.
This rotary tool has a removal battery that also doubles as a hand grip. However, I noticed that the battery cannot be locked to the tool properly. This is quite dangerous as the rotary tool runs at 20K RPM and some of the tools are used for cutting wood and metal. Depending on how you grab the tool, when the battery come loose, the top portion could come off and cause...
