  • Incident Description: The consumer is filing a report on all pacifiers distributed by the firm. The consumer received 4 replacement pacifiers on 02/11/15 from the firm. The consumer believes these pacifiers pose a safety hazard and should be recalled. The consumer states the part of the pacifier that goes inside the mouth of a baby has a plastic piece and it protruded through the silicon material. This occurred with 2 of the replacement pacifiers that the consumer received from the firm. Once the consumer noticed she took them away from her son. On 04/13/15 the consumer noticed that the plastic piece on the 3rd replacement pacifier had cut her son on the cheek. On 04/22/15 the consumer contacted the firm and informed them of the incident. The consumer spoke with the [REDACTED], [REDACTED] who offered the consumer replacement pacifiers. The consumer is more concerned with the safety of the pacifier and expressed to [REDACTED] that the design is dangerous. [REDACTED] told the consumer that the firm is looking to change the design of the pacifier.
