- Incident Description: Mam pacifier nipple came unattached from shield.
Mam child pacifier
12mth old using product as a pacifier for sleeping
Pacifier nipple was bit and detached from shield. If child had been asleep they could have swallowed the nipple and could have potentially suffocated in his sleep.
Pacifier had been replaced just two days prior due to small cut along nipple and shield base in previous pacifier.
No injury occurred but now am fearful to recommend this brand of pacifier to anyone no will I purchase again.
Contacted company and was given information on how to use and when to replace.
