The 6-MO was sucking on the pacifier when she turned it sideways and it got completely in her mouth. The pacifier then flipped the “right way” so the pacifier back was flat against the inside of her mouth. She began gagging and clawing until mom pried it out of her mouth.

  • Incident Description: My child (6 months old) was sucking on her soothie pacifier, she turned it sideways and got the pacifier completely in her mouth. The pacifier then flipped the “right way” so the pacifier back was flat against the inside of her mouth. She began gagging and clawing at her face to attempt to remove the pacifier- she could not and I prayed it out of her mouth.
