2 pacifiers of 9 MOM snapped in half leaving circular end detached from pacifier. Both times consumer caught it before infant could grab piece & put it in his mouth. Consumer stated that when pacifier hits hard floor it breaks connection between circular piece & pacifier.

  • Incident Description: No injuries were sustained, but the Nuk Design OrthoStar pacifier poses a huge choking hazard. TWO of my child's OrthoStar pacifiers have snapped in half leaving the circular end (the size of a quarter) detached from the pacifier. The first time I thought it was a fluke, after two times we will be getting rid of these pacifiers. Luckily both times we caught it before my infant could grab the piece and put it in his mouth. We have had these pacifiers for about five months. The pacifier is made of hard plastic and when it hits a hard floor (ie ceramic tile, wood) it breaks the connection between the circular piece and the pacifier, allowing the circular piece to dislodge. We have only washed these pacifiers in warm water, no dishwasher. So the plastic shouldn't have been weakened.
