When possible, and at the consent of the consumer involved, product samples are requested for review to determine if there are any steps that we can take to prevent a recurrence. Where product information is provided that allows the specific machine to be identified, quality systems information for the lot are reviewed. When detailed product information is unavailable, investigations are focused on the expected equipment and processes where the reported products were made.

When this report was shared with us, we reviewed the contacts from this consumer dating back to 2001. At the time of the alleged incident, the consumer did not contact us to report this situation and therefore neither samples nor specific information about the lot of product were available for investigation.
