    Incident Description: Was cutting A piece of wood when the table saw stand dropped suddenly flinging the piece into my face. It left a piece of wood in my forehead that is now scarred and another piece broke a tooth out of my mouth.

    Incident Description: On several occasions the vacuum cleaner started by itself - once during the day , and twice in the middle of the night. The cord was plugged into the wall plug. In all instances there were people home to unplug the vacuum cleaner.
